Macular degeneration treatment has advanced dramatically over the past two decades. What was once a disease that led inevitably to severe central vision loss can now often be stabilised or even improved with modern therapies. The key is early detection and prompt treatment.
Treatment Depends on the Type
Dry AMD and wet AMD require different approaches. Dry AMD progresses slowly and has no specific cure, but nutritional and lifestyle interventions slow progression. Wet AMD is more aggressive but responds remarkably well to anti-VEGF injections.
Dry AMD Treatment
The AREDS2 vitamin formulation has been shown to reduce the risk of progression to advanced AMD by about 25 percent in patients with intermediate disease. This formula includes high-dose vitamin C, vitamin E, lutein, zeaxanthin, zinc, and copper.
Lifestyle modifications matter. Stop smoking — this is the single most important action. Eat plenty of dark leafy greens and coloured fruits. Wear UV-protective sunglasses. Manage blood pressure, cholesterol, and weight. Exercise regularly.
Monitoring is important. Home Amsler grid testing can detect conversion from dry to wet AMD. Regular clinical exams with OCT detect changes before significant vision loss.
Wet AMD Treatment
Anti-VEGF injections are the standard of care. Medications like ranibizumab (Lucentis), aflibercept (Eylea), and bevacizumab (Avastin) are injected into the vitreous — a quick, in-office procedure with topical anesthesia.
Initial treatment typically involves monthly injections for 3 months, followed by ongoing injections based on response. Many patients can stabilise on injections every 2 to 3 months. About 30 to 40 percent of patients experience some vision improvement, and most stabilise.
Photodynamic therapy and laser treatment are less commonly used today but may be appropriate for select cases.
Advanced Cases
Research continues into new treatments — gene therapy, sustained-release implants, stem cell therapies, and combination treatments. Patients in clinical trials may access these options.
